چکیده
Gastric perforations are usually intraperitoneal, producing classic symptoms and signs. Perforation into adjacent organs, such as the colon, small bowel and aorta have been reported. Gastric ulcers in hiatus hernias have the potential to erode into mediastinal and thoracic structures such as the aorta, lung and bronchi. Penetration directly into the heart is very unusual. We describe a case of a fatal perforation of an ulcer in an hiatus

Management of oesophageal perforation associated with benign stricture and hiatus hernia by oesophagoplasty and fundoplication.
The operation of oesophagoplasty and Nissen fundoplication has proved valuable in the management of intractable peptic stricture of the oesophagus. This report concerns its use in perforation of the oesophagus in an 81-year-old woman with benign peptic stricture in whom perforation occurred during instrumentation. She made an uneventful recovery from operation and remained well and able to swal...
متن کاملSudden death from perforation of a benign oesophageal ulcer into a major blood vessel.
Two cases of sudden death due to perforation of a benign oesophageal ulcer into a major blood vessel are reported. In one man, anaemia and aspiration pneumonitis dominated the clinical picture. He had an oesophageal stricture and a chronic peptic ulcer associated with an incarcerated hiatus hernia. Death was due to haemorrhage caused by perforation of the ulcer into the thoracic aorta. The seco...

The endoscopic diagnosis of hiatal hernia (sliding type) relies on direct and indirect criteria during the oesophagoscopic or gastroscopic approach. A wide separation between the anatomic and mucosal oesophago-gastric junction, the presence of a so-called Schatzki-ring, a wandering junction with sliding gastric mucosal folds during inspiration and two ringlike structures are important. Whereas ...
متن کاملPenetration of the left ventricular myocardium by benign peptic ulceration: two cases and a review of the published work.
Two cases of penetration of the left ventricular myocardium by benign peptic ulcer are reported. Twenty five similar cases in the world published work are reviewed. The condition is only possible when there are fibrous adhesions between the stomach and diaphragm and the pericardium. In addition, the left lobe of the liver may be small. Alternatively, an ulcer within a hiatus hernia may erode in...
